| How do I apply for a Credit Account? |
You will need to download the CREDIT APPLICATION form. Your signature is required in two places as indicated. You will need to provide any job information and other misc. facts. Fill out the form with your mailing address, phone - fax - email numbers. List your bank accounts and include your social security number. We require the original forms for our files, but to get the process started, you can fax both pages of the credit application to 760-747-7094.
